Enrollment Requirements

The following
information must be turned in and approved before a student may begin attending school.
 
1.     Enrollment packet (which includes)
            -Application Form
            -Signed Commitment Statement (from all parents and students)
            -Completed Student Information Form (parents & students grade 5 & up)
            -Completed & Signed Web & Media Permissions
            -Signed Behavior Contract (from all parents & students in grades 5 and up)
            -Immunization and Medical Form (all new students and students entering grade 10)
    -Field Trip & Continuing Consent Forms
            -Financial Plan Form (must be turned in and approved before beginning school)
            -School Shirt Order Form (next year’s color has not yet been determined)
2.     A copy of the student’s birth certificate (all new students).
3.     The entrance fee.
 

The following is the 2010 summer payment schedule:
 
June 30    One–Half Registration Fee Due
K = $147.50
1-2 = $155.00
3-4= $156.25
5-6 = $157.50
7-8 = $167.50
9-10 = $187.50
 
July 20   Class Supply, Class Trip, & Other Fees Due
K half = $35.00
K-4 = $60.00
5-6 = $50.00 + $125.00 for outdoor school, if not participating in fund-rasing
7-8 = $80.00
9-10 = $110.00
                        10 = additional $40.00 ELS fee
 
August 5   Remainder of Registration Fee Due
K = $147.50
1-2 = $155.00
3-4= $156.25
5-6 = $157.50
7-8 = $167.50
9-10 = $187.50
